Securing Mesh with Delayed Absorbable Suture Does Not Increase Risk of Prolapse Recurrence After Robotic Sacral Colpopexy

چکیده

Introduction and Objective: Nonabsorbable monofilament suture is traditionally used to secure vaginal mesh for robot-assisted sacral colpopexy (RASC) but can become exposed postoperatively requiring invasive removal. Polydioxanone delayed absorbable may avoid this. We sought determine the objective subjective impact of choice fixation. Materials Methods: A cohort study was undertaken using a prospective registry subjects were grouped based on type at time RASC. Apical failure defined as C point descent >2 cm, anterior compartment pelvic organ prolapse quantification (POP-Q) Ba >0, posterior Bp >0. Patient-reported outcomes included urogenital distress inventory (UDI)-6 QoL. Two-tailed t-test chi-squared analysis. Results: total 119 women underwent RASC between 2009 2016. Patients had similar preoperative characteristics (Table 1). All POP-Q, UDI-6, quality life (QoL) scores improved (Tables 1 2). noted in 0, 7 (average +1.1 cm failures), 4 (mean +1.0 cm) 16 months' follow-up. Failures much more common nonabsorbable apical compartments not significantly different groups. Nine erosions cohort, five excision clinic two operating room. Two 0 required excision. Postoperative UDI-6 QoL did vary groups (5.3 ± 4.0 vs 5.1 4.0, p = significant (NS), 2.8 2.0 2.2, NS). Table 1. Preoperative Demographics 2. Outcomes Sacral Colpopexy by Suture Type Conclusion: Securing with appear increase risk patients undergoing eliminates need postoperatively.

Sacrospinous ligament suspension with transobturator mesh versus sacral colpopexy for genital prolapse

OBJECTIVE To compare the safety and efficacy of abdominal sacral colpopexy and sacrospinous ligament suspension with the use of vaginal mesh for apical prolapse. METHOD This retrospective study was conducted from 2005 to 2012 and included 89 women with apical prolapse who underwent surgery. Assessments included pre- and postoperative Pelvic Organ Prolapse Quantification (POP-Q) stage. Rates o...

Objectives We sought to describe the perioperative and postoperative adverse events associated with sacral colpopexy and evaluate the surgical outcome, complications, and benefits of laparoscopic sacral fixation for patients with pelvic prolapse. Methods Ninety-two women with uterine prolapse underwent sacral colpopexy between January 2011 and September 2016 at Chosun University Hospital. Pat...

Laparoscopic sacral colpopexy versus total vaginal mesh for vaginal vault prolapse: a randomized trial.

OBJECTIVE To compare the laparoscopic sacral colpopexy and total vaginal mesh for vaginal vault prolapse. STUDY DESIGN Women with symptomatic stage ≥2 vault prolapse were randomly allocated the laparoscopic sacral colpopexy (53) or total vaginal mesh (55). Primary outcome measures were objective success rates at pelvic organ prolapse quantification sites individually and collectively. Seconda...

Abdominal sacral colpopexy or vaginal sacrospinous colpopexy for vaginal vault prolapse: a prospective randomized study

Study sample Power calculations were performed in the preliminary phase of the study. These were based on a 58% success rate for the abdominal group, a 29% success rate for the vaginal group, a significance level of 5% and a statistical power of 80%. This led to a sample size of 40 women in each group, which was increased to 95 to minimise the impact of loss to follow-up (expected to be around ...
